Thank you for your interest in attending a FLORIDA MOONSHINE spanking party. Before you can do so, you must answer these questions. EACH applicant to FLORIDA MOONSHINE is REQUIRED to truthfully fill out this form before they can be considered to attend a party or to become a member of the club. For the safety of our guests, your real identity WILL be verified (you may keep it a secret from other guests if you wish, but FLORIDA MOONSHINE Leadership has to know).

Carefully fill out your answers. They are our first impression of you! Applications with one or two lines will not be accepted.

Please use the address on your driver's license (even if it is from another state) so we can verify your identity quickly and easily.

Please list the email address you use on our Yahoo Group and use this same email address in all your correspondence with us. This helps avoid confusion if we need to answer questions or locate your records.

All information provided will be held in the strictest confidence. Our mailing list is not sold or traded to anyone. We will exercise discretion in all our mailings to you, and expect the same in return.

BY SUBMITTING THIS FORM, I HEREBY HOLD HARMLESS FLORIDA MOONSHINE AND ITS LEADERS, FROM ANY LOSS, DAMAGE OR INJURY WHICH MAY OCCUR AS A RESULT OF MY ATTENDANCE OR PARTICIPATION IN ANY EVENT SPONSORED OR HOSTED BY FLORIDA MOONSHINE. I ALSO CERTIFY THAT I HAVE READ, UNDERSTOOD AND WILL ABIDE BY THE CODE OF CONDUCT WHICH GOVERNS ALL THOSE WHO ATTEND FLORIDA MOONSHINE PARTIES. I FURTHER CERTIFY THAT I AM AT LEAST 21 YEARS OF AGE. Photo ID will be checked at participant's first party.

(This form will be submitted to us by email. We will have a printed copy of it for you to sign when you attend your first FMS party.)
